We do not ask you to send us medical history, prescriptions, eye records, or other private health details. The real medical questions happen later with the surgeon's office, if you decide to schedule.
This matters for a simple reason: only a licensed eye surgeon, after an in-person exam, can say whether you may be a candidate and discuss what fits your eyes. Many people are not candidates, and an honest surgeon will say no.
What happens after you submit the form
Here is the usual process:
- You send basic contact details. We use them to look for licensed ophthalmologists near you.
- We share possible consultation options. Depending on your area, language needs, and what procedure you asked about, there may be one or more options.
- You decide whether to contact or schedule. There is no obligation to move forward.
- The surgeon's office handles the exam. That is where they check your prescription stability, corneal shape and thickness, eye health, age-related changes, and other factors.
- You compare and choose. You can meet more than one surgeon. You can ask questions. You can walk away.
No surgery happens just because you filled out a form. No one can honestly tell you the best procedure for your eyes without an exam first.
